You can fill out the form by hand and deliver to Lee & Gail.
But, you can not get your passes right away.

These Forms are GIF pictures.
To Print Any of these Forms, go to your 'File' &
'Page Setup'
Set all Margins to 0", turn off
Headers and Footers.
Then it should Print a full page.
If you need a Game Day Line-Up Form,

When the form comes up you can print it.
If your Print margins are opened up, it should print a full page form.
It ain't perfect, but it works!
